Summary "IOM's 1999 landmark study To Err is Human estimated that between 44,000 and 98,000 lives are lost every year due to medical errors. This call to action has led to a number of efforts to reduce errors and provide safe and effective health care. Information technology (IT) has been identified as a way to enhance the safety and effectiveness of care. In an effort to catalyze its implementation, the U.S. government has invested billions of dollars toward the development and meaningful use of effective health IT. Designed and properly applied, health IT can be a positive transformative force for delivering safe health care, particularly with computerized prescribing and medication safety. However, if it is designed and applied inappropriately, health IT can add an additional layer of complexity to the already complex delivery of health care. Summary This monograph serves as a technical source for National Institute for Occupational Safety and Health (NIOSH) incident commander guidelines for emergency response immediately following large structural collapse events. It characterizes response activities and expected hazards, and develops guidelines for selecting appropriate personal protective equipment (PPE). The partial or complete collapse of a multistory building creates an array of physical, chemical, and biological hazards. The most significant uncertainties are the composition and magnitude of the hazards present in the postcollapse environment. Although hazard monitoring can reduce these uncertainties, this type of monitoring will not be available during the first few postcollapse hours. The guidelines focus on three issues that present unique challenges in the response to a multistory-building collapse: (1) protection from biological hazards; (2) protection from inhalation of hazardous materials; and (3) required modifications to responders' typical PPE ensembles because of the duration and intensity of the response. However, selecting and purchasing appropriate PPE does not alone ensure safety; the guidelines also address the need to provide adequate supply of equipment and training so that it is used correctly.

The authors developed a framework to guide assessments of the availability of such information for planning attacks on the U.S. air, rail, and sea transportation infrastructure, and applied the framework in an information-gathering exercise that used several attack scenarios. Overall, the framework was useful for assessing what kind of information would be easy or hard for potential attackers to find. For each of the attack scenarios, a team of 'attackers' was unable to locate some of the information that a terrorist planner would need to gauge the likely success of a potential attack. Summary Clinics have dealt with a range of victims' rights issues in trial courts, including the rights to be present, to be consulted about plea offers, to make an impact statement, to be notified of changes in defendants' detention status, to restitution, and to privacy. However, the principal issue has been victims' standing before the court to enforce their rights. In some states, standing has been acknowledged, at least in limited ways. In other states, clinics have made or are making steps toward such recognition or have been successful in representing victims without the issue being directly confronted. In one state, attorneys' ability to represent victims in criminal court is currently in serious question. This book discusses how some clinics have won significant gains at the appellate and federal court levels concerning victim standing, the rights to be consulted and heard, and the right to privacy. In 2007, on a net basis, the United States imported 58 percent of the oil it consumed. This book critically evaluates commonly suggested links between these oil imports and U.S. national security. The major risk to the United States posed by reliance on oil is the economic costs of a major disruption in global oil supplies. On the other hand, the study found no evidence that oil exporters have been able to use embargoes or threats of embargoes to achieve key political and foreign policy goals. Summary The aim of this book is to document, on a solid and convincing foundation, two public policy mistakes of the United States Government that have been extremely costly. First, the failure to combine stocks with long-term government bonds in the Social Security Trust Fund, the way other nations do, has resulted not only in an investment shortfall well into the trillions of dollars, but has also reduced US and global economic growth and increased the national debt. Second, by employing the Unified Budget concept beginning in 1970, the US Government has since then understated its financial deficits by more than $4 trillion and in doing so it has shielded the increase in the debt owed to the public by roughly half. This study documents that the notion of Social Security as a minimal safety net is consistent with the views of both Adam Smith and Friedrich Hayek and that private social security accounts are inefficient and subject to moral hazard and huge productivity losses. It also introduces a novel approach to long-term investing suitable for perpetual funds consistent with the empirical phenomena of risk premia and mean reversion, including no asset sales and the use of short-term borrowing on a rollover basis to cover negative net inflows. The study also proposes that payroll taxes be re-labeled Social Security Contributions and that the Social Security System be made independent and professionally managed based on the Federal Reserve System model.

Summary Family foster care is supposed to provide temporary protection and nurturing for children experiencing maltreatment. Although it has long been a critical service for millions of children in the United States, the increased attention given to this service in the last two decades has focused more on its inability to achieve its intended outcomes than on its successes. However, as social and political trends and new legislation reshape child welfare, policymakers and service providers continue to offer innovative policy and practice options for this child welfare service. Though use of the service has changed, family foster care remains important. Responding to a widespread sense of the "drifting" of children in care, Congress passed the Adoption Assistance and Child Welfare ACt of 1980. This legislation became a key factor shaping the current status of family foster care. Its goal was to reduce reliance on out-of-home care and encourage use of preventative and reunification services; it also mandated that agencies engage in planning efforts for permanent solutions for foster children. Yet despite federal mandates and funding, the child welfare system has continued to struggle to provide the level of services needed for children to reduce the amount of time children remain in temporary foster care. The latest response to these problems, the Adoption and Safe Families Act of 1997, established unequivocally that safety, permanency, and well-being were national goals for children in the child welfare system. To comply with the law, public and private agencies are required to initiate significant program and practice changes in the coming years to improve permanency outcomes and child well-being in family foster care. All the major sights in the destinations covered are described, but so are little dumpling houses, lively markets, walks through narrow old streets, places to study tai chi.
